• What should be done in case of an emergency?
• Where are the pet's favorite toys? 
• Is your dog a chewer?  Where are its chew toys? 
• Where are the pet's favorite hiding places? 
• Does your pet have any unusual habits or fears? 
• Provide updated verification of vaccinations (collar tags or copy of vaccination certificates)
• Ensure the pet has an identification tag with name, address, and phone number.
